What is a UI-8 form?

The UI-8 form is an official declaration submitted to the Unemployment Insurance Fund (UIF) when registering a new employer. It provides key company details, including business registration number, contact information, and banking details. This form is submitted alongside the UI-19 form when registering employees for UIF.

The UI-8 form is required by the Department of Employment and Labour to onboard your business into the UIF system and ensure that both employer and employee contributions can be properly tracked.

Who needs to submit the UI-8 form?

All South African businesses that employ staff and are liable for UIF contributions must submit a UI-8 form. This includes:

  • Companies (Pty Ltd)
  • Sole proprietors and partnerships with employees
  • Non-profits and co-operatives

If your business pays employees more than 24 hours per month, you are legally required to register for UIF, and the UI-8 is the first step in that process.

Where do I get the UI-8 form?

You can:

  • Download it directly from the Department of Labour website
  • Collect it from a Department of Labour office
  • Use a service provider like Govchain to complete and submit the form on your behalf

It’s usually submitted together with the UI-19 (employee details) and proof of company registration.
